Office Policies
Patient Responsibility
​
Pets: are not allowed, only service animals.
​
Cancellation fee: will be processed for appointments that are canceled with less than 24 hours notice.
​
Late arrival to appointments: there will be no extension of the allotted time for showing up late to an appointment (telemedicine or an in-person visit)
​
Co-pays/outstanding balances: are due at the time of service.
​
Returned check fee: $50
​
Additional professional fees: if a patient requires the provider to participate in legal proceedings despite the provider being called to testify by another party, the patient will be responsible for professional fees which include professional time for preparation, transportation, and additional legal costs as lawyer fees that the provider may incur: $50/15 minute increments.
​
Medication refill policy: we do not provide refills outside of business hours, weekends, or holidays. A follow-up visit will be required at least every three months for non-controlled substances.
​
Miscellaneous: we do not offer disability psychiatric evaluations, accommodation letters for work/school, completion of paid family medical leave, or driver’s license reinstatement forms for new patients, and/or for patients who haven’t been under the care of Vibrant Minds and Wellness, PLLC for at least 6 months.
​
Emails: are ONLY monitored once a day during business hours, and are NOT intended to communicate/notify of an emergency. Please allow up to 3 business days for a response.
​
Electronic health record communication: is not intended to communicate/notify of an emergency, please allow up to 24 hours for a response during business hours from Monday to Friday.
