Controlled Substance Policy

Patients who are prescribed controlled substances require an appointment for medication refills.
We don't provide any early refills for lost or stolen controlled substances.
We also don't provide early refills for controlled substances due to travel.
Patients will meet with the provider regularly to assess progress as designated by the provider.
Agree to random urine screens and in-office pill counts.
The prescriber may discontinue controlled medications if the terms of the agreement are not met.
As for new patients, the prescriber will only continue controlled medications if he/she deems it clinically indicated.
